After adopting Claude Code or Codex, teams quickly hit the same wall.

Agents always start from a blank slate. Architecture decisions you spent half a day explaining to the Agent last week are completely forgotten when you start a new session this week.

You have to re-explain the project background, re-state the conventions, and re-step through the same pitfalls you already overcame. The time cost remains high, and the Agent doesn’t get any smarter.

CLAUDE.md becomes useless once written. You carefully wrote a specification document at project kickoff, but three months later, the project has evolved, and no one updated the document, and the Agent continues generating code based on the outdated rules. Even worse, no one knows which rules are still valid and which have been overturned.

All knowledge in conversations evaporates. You spent twenty rounds debugging a tricky bug with the Agent and finally identified the root cause. The implicit rules that emerged during this process are never turned into documentation. The next time a similar problem appears, you have to start over.

Multiple people, multiple versions of the truth. Developer A agreed on an error-handling convention with the Agent, but Developer B doesn’t know about it. The Agent then produces code with inconsistent styles for the two developers. No one notices until code review.

The root cause of these four pain points is the same: the team’s knowledge is scattered across individual chat histories, with no single place where the Agent can always work with the same context for everyone, at any time.
